SERVICE DOG & EMOTIONAL SUPPORT ANIMAL EVALUATIONS

A trained service animal will perform a service relative to the psychiatric or physical condition that the user has with a required prescription. 

Fully-trained service dogs may fly in the cabin at no charge if they meet the requirements.  A service animal is defined as a dog that’s individually trained to do work or perform tasks for the benefit of a qualified individual with a disability, including a physical, sensory, psychiatric, intellectual, or other mental disability.

Rate: $500

Expedited Rate: $600 (Within 24 hours of the completed appointment)

The renewal prescription renewing prescriptions specifically for service dog patients who are already under our care and whose initial prescriptions are approaching their expiration dates. 

To qualify for this renewal, the patient must be an existing service dog recipient with an existing medical record in our system and their current prescription nearing its expiry date.

To facilitate the prompt processing of the prescription renewal, you must submit the up-to-date vaccination records and registration documents.  clients are required to submit the following updated documents.

Expedited Rate: $300 (Within 24 hours of completed appointment)

Airlines discontinued recognizing emotional support animals as of January 11, 2021. Consequently, they are now treated as regular pets. ESA letters are invalid for airline travel.

Upon completion of a consultation or appointment to assess the individual's need for an emotional support animal. This letter is intended to provide relevant parties, such as landlords or housing providers, with information supporting accommodations for the individual and their emotional support animal, as outlined by applicable government regulations, including the Fair Housing Act.
