PATIENT REFERRALS
Medical Practice Patient Referrals
SFPS accepts patient referrals for diagnostic evaluations, inclusive of anxiety, depression, PTSD, OCD, ADHD and bipolar disorder. Our office will only conduct the diagnostic evaluation to provide a clinical diagnosis (if necessary) and recommendations for treatment.
Please note, our office DOES NOT accept insurance or medicare/medicaid. All services are self-pay and require payment in advance of each scheduled appointment.
To submit a referral, please fax your request to (469) 775-9220 and one of our courteous staff members will follow-up on the request.
Legal Referrals
A mental health evaluation can be legally required with a court order or at the request of an attorney. SFPS offers evaluations for the purposes of satisfying the requirements of a court order or to aide in an ongoing legal case. Please reach out to our office to inquire if we can assist with a mental health evaluation for your client.
