Request your health records

As the patient or patient representative, you must reach out to our office directly to request a copy of your records. You can call us at 214-432-4119 or email us at information@sfsppllc.com. Please note, there is a fee to issue to the records.

If the matter requires further escalation, you may reach out to the Attorney General's Office to file a consumer complaint.
